The TypeForm thing was just the site that hosted the sign-up that Taco Bell used. It’s like any generic “make a survey” platform.
If you got your name and email (associated with your Taco Bell account) in, and got the above screen, you were done.
I’m guessing that the list will be retrieved by Taco Bell, and they will match it against their database and send an email to all the matches. Not sure what they will do with emails that were not already attached to an account.
